Registration Types
- ● Regular Conference Registration: This includes: conference materials, lunch for 5 days and dinner for 5 evenings including the social dinner.
- ● Two-day Registration: This includes: conference materials, lunch for the 2 days attending the conference and dinner for the first evening.
- ● If you would like to bring a guest to the social dinner an "Additional Person Fee for Meals and Social Event" may be purchased. The Additional Person Fee for Meals and Social Event includes dinner for 4 evenings and all social activities.

- ● Register by Mail with a check or Register by fax with a major credit card: Visa or MasterCard Only.

Send full payment in US dollars with this form. Use a check drawn on a US bank or a major credit card. For payments from non-US banks, the attendee will be charged a collection fee of US$45. Purchase orders are not accepted. Use your credit card if registering by fax.

Refund/Cancellation Policy
Written requests for cancellations must be received on or before May 31, 2018, and are subject to a $50 processing fee. Cancellations received after May 31, 2018, will NOT be honored and all registration fees will be forfeited.
